The immigration medical exam – medical examination is needed by those who apply for Form I – 485: Lawful Permanent Resident status in the US. They will have to identify an authorized Doctor nearby to them for undergoing the immigration medical exam.
Applicants who are in the US must undergo the immigration medical exam through a USCIS designated Physician. The person is also known as ‘Civil Surgeon’.
The applicants of for Form I – 485: Lawful Permanent Resident status outside the US must undergo the exam conducted by Physician. This is somebody who is designated by the Department of State. This Physician is also known as ‘Panel Physician’, as quoted by the USCIS Gov.
The fee charged by the Civil Surgeons for completing the medical examination is not regulated by the USCIS. The rates are variable depending on the Physician.
You must bring a print out of the latest version of the Report of Medical Examination and Vaccination Record: Form I – 693. This is for the appointment to the medical exam. You must complete Part 1 of this form. Until the Civil Surgeon instructs, you must not sign the form. This form will also be used by the Civil Surgeon for documenting the results of your medical exam.
The Form I-693 will be completed, signed, and sealed by the designated Civil Surgeon along with any supporting documents in an envelope. This must be submitted to the USCIS as directed in this form itself.
The envelope must not be opened nor must its seal be broken. If the envelope is not sealed or altered in any manner, USCIS will not accept the Form I-693.
